Recognising the inadequacy of her environment for her actual needs, we intervened, crafting a personalised care package to relocate Martha to a community setting closer to her family.

Peter and Jean share the transformative journey of their child, C, who, in their early 20s, spent five challenging years in various hospitals far from home.
